Family Home Case Manager

Bachelor's degree from an accredited college in social work, psychology, or related field required.
Must have two years of cumulative experience that includes counseling with youth, crisis intervention, intake and assessment, and/or case management.
Provides expedient and ongoing case management, family preservation, and transition planning for all Family Home youth. Duties include, but are not limited to, preparing and submitting all required paperwork, reports, and statistical data; coordinating the collaboration of referral agencies, family members, and other support systems to meet the needs of youth in care. Must be able to effectively communicate with the team the progress and service plans of each youth. |
